The area directly beneath the eye, known as the tear trough, is often the first part of the face to show signs of tiredness and ageing. Hollowing, dark circles, and shadowing can leave us looking perpetually exhausted, even after a good night’s sleep. Correcting this area with dermal filler is one of the most transformative aesthetic treatments available, yet it is also one of the most technically demanding.

Why the Tear Trough is a Specialist Area

Unlike other areas of the face, the skin under the eyes is exceptionally thin, often compared to the thickness of tissue paper. This makes the area highly unforgiving:

  1. Delicate Anatomy: The skin is close to underlying bone, and the fat pads that cushion the eye are extremely sensitive. Key blood vessels and nerves run through this region.
  2. High-Risk for Poor Results: If the wrong product is used, or the filler is placed too superficially, it can lead to noticeable issues. These include lumps, prolonged swelling, or the notorious Tyndall Effect—a bluish discolouration caused by light reflecting off filler placed too close to the surface.
  3. Need for Precision: Treating this area requires not only precise technique but also a deep understanding of periorbital ageing (ageing around the eye), which often means treating the cheek area first to provide necessary underlying support.

The Doctor-Led Difference: Safety in Technique

This is not a treatment for inexperienced practitioners. A medical professional, especially one with extensive injectable experience, employs specialist techniques to maximise safety and efficacy:

  • Cannula Technique: For tear trough correction, we almost exclusively use a micro-cannula—a blunt-tipped instrument—rather than a sharp needle. The cannula pushes past blood vessels and nerves instead of cutting them, dramatically reducing the risk of vascular injury and severe bruising.
  • Product Selection: The filler used in the tear trough must be specifically formulated for this delicate area. It needs to be very smooth, flexible, and have a low hydrophilic (water-attracting) property to minimise swelling and puffiness post-treatment.
  • Holistic Assessment: A successful tear trough result often starts with the mid-face. If the cheek has lost volume, the under-eye area loses support. Dr. Noreen always assesses the entire cheek-to-eye area, sometimes recommending volume restoration in the cheeks before treating the trough, creating a smoother, more supported transition.

Realistic Expectations are Key

While tear trough filler can be life-changing, it is essential to have realistic expectations. The goal is to correct hollowing and reduce shadowing, not to eliminate dark circles caused by pigmentation or excessive skin.
